SASMI stands for the National Stabilization Agreement of the Sheet Metal Industry Trust Fund. SASMI provides financial and benefit support to eligible SMART members who permanently separate from covered employment due to retirement, disability, death, or other qualifying circumstances.

SASMI benefits may include severance benefits, health care reimbursement accounts (HCRA), retiree health premium assistance, training benefits, and death benefits. Eligibility, benefit amounts, and application requirements are governed by the SASMI Plan Rules and Regulations and administered by the SASMI Trust Fund.

SASMI is designed to help stabilize members’ financial security during major career transitions while maintaining compliance with federal benefit and tax regulations.

SASMI HCRA Reimbursement Request Form #

This form is used by eligible SASMI participants to request reimbursement for qualifying out-of-pocket health care expenses. Reimbursable expenses may include medical, dental, vision, prescription, and insurance premium costs that are not fully covered by other plans. Claims must meet IRS requirements, include proper documentation, and be submitted within the applicable time limits.

SASMI Severance Benefit Beneficiary Designation Form #

This form allows SASMI participants to designate primary and secondary beneficiaries for their SASMI Severance Benefit. In the event of a participant’s death before severance benefits are paid, benefits will be distributed according to the beneficiary designations on file. Keeping this form current helps ensure benefits are paid according to the participant’s wishes.

SASMI Severance Benefit Permanent Separation Application #

This application is required for participants seeking SASMI Severance Benefits following permanent separation from the sheet metal industry. Qualifying separation reasons may include retirement, permanent disability, death, or individual withdrawal. Submission of this application does not guarantee benefits; eligibility is determined based on SASMI Rules and Regulations and supporting documentation.
